These conditions exposed employees to struck-by and health hazards.    GSA was notified on 04/04/2017, 04/17/2017 and 04/20/2017 of the conditions listed below:   a) Visible mold residue was found in several places including ceiling tiles, desks, and ceiling vents.   b) Ceiling tiles were broken and not secured.
