29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:    (a)  Campos Masonry, Inc. at 3 Superior Drive, Superior, CO 80027:  On and before May 15, 2017 the exposing employer did not ensure the working platforms on the fabricated frame scaffold surrounding the building under construction were fully planked in that two to three employees were performing masonry (block) work from the third level of the scaffolding while it was not fully planked in at least three different areas. This condition exposed the employees to a fall of approximately 20 feet.

29 CFR 1926.451(g)(1):  Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level:     (a)  Campos Masonry, Inc. at 3 Superior Drive, Superior, CO 80027:  On and before May 15, 2017 the exposing employer did not ensure that employees were protected from a fall from elevation, in that guardrails or other suitable fall protection were not properly installed on the work platforms of the scaffolding.  Two to three employees were performing masonry (block) work from the third level of the scaffolding where both exterior toprails and midrails were not installed in two sections.  This condition exposed employees to fall hazards of approximately 20 feet.

29 CFR 1926.451(h)(2):  Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:      (a)  Campos Masonry, Inc. at 3 Superior Drive, Superior, CO 80027:  On and before May 15, 2017 the exposing employer did not ensure that employees on the ground were protected from falling tools, materials or equipment, in that toe boards or other suitable provisions were not implemented. Two to three employees were performing masonry (block) work from the third level of the scaffolding.  This condition exposed the employees working on the ground to struck-by hazards caused by falling objects.

This condition exposed employees to a fall of approximately 20 feet.

29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration:      (a)  Campos Masonry, Inc. at 3 Superior Drive, Superior, CO 80027:  On and before May 15, 2017 the exposing employer did not ensure a fabricated frame scaffold was erected with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration.  Employees assembled a scaffold system that was up to four levels high on all sides of the building without the supervision of a competent person qualified in scaffold erection, moving, dismantling or alteration. This condition exposed employees to fall hazards of approximately 20 feet.
