29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: At the location of 11640 Metcalf Ave in Overland Park KS, an employee engaged in operating a scissor lift from the middle of the facility to the south of the facility was exposed to falling onto the concrete below while sitting on the top rail. The top rail was found to be less than 18 inches wide.

29 CFR 1926.451(g)(4)(iii): Midrails were not installed midway between the top rail and the platform surface At the location of 11640 Metcalf Ave in Overland Park KS, an employee engaged in operating and working from a scissor lift at the south of the facility was exposed to fall hazards greater than 10 feet onto the concrete below in that the middle rail at the end of the scissor lift was not installed.
