29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) 9980 Trainstation Circle: On or about May 16, 2017, and at times prior, workers without personal fall protection were exposed to fall hazards when one worker was on the third floor deck at the immediate edge of a 22 foot fall to ground level and three workers were walking top plate and/or the tops of open web wood joists.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a) 9980 Trainstation Circle: On or about May 23, 2017, and at times prior, a worker without personal fall protection was exposed to falling 10 or more feet to ground level while sweeping accumulated material into an unprotected open hole in the second level deck.  Abatement Note: Abatement completed during inspection.

29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels:  a) 9980 Trainstation Circle: On or about May 16, 2017, and at times prior, workers were exposed to hazards from falling objects at various locations where toe boards were not installed in areas with loose material and construction debris positioned at the immediate edge of decking ten to twenty feet above lower working levels. 29 CFR 1926.502(b)(2)(iv): Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used gbetween posts, were more than 19 inches apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches wide:   a) 9980 Trainstation, Bldg 4B: On or about May 23, 2017, and at times prior, workers without personal fall protection were exposed to fall hazards where vertical members used as fall protection were spaced at 22.75 inches and 22 inches at the immediate edge of a fifty foot fall.
