29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: The controlling contractor is failing to ensure that all employees of the sub-contractor Casiqu Enterprises, Inc exposed to fall hazards while working on a commercial roof are utilizing fall protection. This most recently occurred on May 16, 2017 on the jobsite located at 10901 Jaynes Plaza, Omaha, NE 68164. Employees of the controlling contractor were in a position to personally observe the sub-contract employees on the roof of a 3-story apartment with a 6 in 12 steep sloped roof at heights greater than 6 feet above the ground and not utilizing any form of fall protection.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: The general contractor is exposing employees from the sub-contractor Casiqu Enterprises, Inc to fall hazards associated with using faulty personal fall arrest systems (PFAS) while working on a commercial roof. This most recently occurred on May 16, 2017 on the jobsite located at 10901 Jaynes Plaza, Omaha, NE 68164. Sub-contract employees are working on a roof that has a 6 in 12 steep sloped pitch and a ground to eave height of about 30 feet. The general contractor is providing the sub-contractor employees with PFAS that has not been inspected. The PFAS has connectors that are rusted which can cause them to break easily or cause cuts and tears to other components of the PFAS. There are also frayed ropes which can cause the PFAS to not function properly in the event of a fall.
