29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   On or about May 15, 2017 at 1439 Industrial Park Blvd., Washington, Georgia where the controlling employer did not conduct frequent and regular inspections of the workplace to identify and remove electrical hazards including but not limited to:     a.         In Building #2, at temporary electrical panel D, a ground pin was broken off in one of the outlets exposing employees working in the area and using the electrical panel to an electrical shock or electrical burn hazard.       b.         In Building #2, at the temporary electrical panel D, the covers on the junction boxes and the receptacles were not flush or completely closed exposing employees working in the area and using the electrical panel to an electrical shock or electrical burn hazard.       c.         In Building #2, in the electrical room, the sheathing on the plug end of the vacuum cleaner used to clean up had pulled away from the connection, exposing the conductors beneath exposing employees to an electrical shock hazard.     d.         In Building #2, in the main hallway, the temporary electrical cords connected at the electrical panel D, were suspended six (6) feet above the finished floor across the hallway.  The cords provided power to various pieces of equipment, including but not limited to a cellular phone and a scissor lift.  Employees navigating the hallway were exposed to a caught by hazard.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
