On or about 05-16-2017 and times prior thereto, located at U.S.C.G. Base Elizabeth City, 1664 Weeksville Rd. Building #48, Elizabeth City, NC 27909, employee(s) were exposed to unguarded and unprotected fall hazards greater than 14 feet through a hole in the rooftop approximately 5 foot by 20 foot in size. The hole was located on the rooftop of the USCG Fire Department and was for the placement of an HVAC system. Abatement certification required

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and safety monitoring system. (a) Date of Violation: On or about 05-16-2017 and times prior thereto located at U.S.C.G. Base Elizabeth City, 1664 Weeksville Rd. Building #48, Elizabeth City, NC 27909, on top of building #48 Fire Station, employee(s) conducting roofing repair work of installing rubber flashing were exposed to falls from heights approximately 14 feet, 4 inches above the concrete ground level below without any type of fall protection system, fall restraint system, safety net, guardrails, warning system, safety monitoring system, or any combination thereof. Roof Engineering Corporation was previously cited for the same violation of this Occupational Safety and Health Standard, 29 CFR 1926.501(b)(10) , which was contained in OSHA inspection number 1121810, citation number 1, item number 1 and was affirmed as a final order on 07-15-16, with respect to a workplace located at Norfolk, VA.
