On or about May 17, 2017, the employer did not remove from service the Caterpillar propane forklift (SN#GEMOO836) that did not have an operable warning device to include back up alarms and/or sirens and a leaking propane line.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about May 17, 2017, the employer did not provide training or information to employees working with hazardous chemicals on the details of the new hazard communication standard which encompasses the Global Harmonization System (GHS).
