29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about May 4, 2017, and in times prior thereto, workers erecting concrete forms were exposed to the hazard of a 17 foot high soil bank collapsing on them. The forms were located in the back of the construction site.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: On or about May 4, 2017, and in times prior thereto, concrete rebar ends were not protected by an approved method. This exposed concrete workers to a hazard of impalement at the construction site.
