29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about May 17, 2017, during the replacement of a fire hydrant at approximately 2660 Euclid Heights Blvd, Cleveland Heights, Ohio, the employer did not ensure employees working inside a trench, with a varying depth of approximately 5.8 to 6.5 feet deep and with near 90 degree vertical walls, were protected from cave-ins by an adequately designed protection system. The soil would have been classified as; Cohesive Soil, type B .
