29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a. At the workplace located at 102 N. Sundale Road, Norwich, Ohio, the employer failed to provide authorized employee training to employees engaging in servicing and maintenance activities, such as (but not limited to) tab machine unjamming and adjustments.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a. At the workplace located at 102 N. Sundale Road, Norwich, Ohio, the employer did not ensure employees implemented energy control procedures prior to unjamming and making adjustments to tab machines. The machines were not isolated from all energy sources prior to engaging in these servicing and maintenance activities.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from the unexpected energization, startup, or release of stored energy during machine servicing and maintenance activities.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  a.  At the workplace located at 102 N. Sundale Road, Norwich, Ohio, employees operated a Herhammer wick tabbing machine, that had a three and a half inch diameter pulley not guarded, thereby exposing the employee to a caught-in hazard.  The pulley was approximately 38 inches from the floor.    b.  At the workplace located at 102 N. Sundale Road, Norwich, Ohio, employees operated a Herhammer wick tabbing machine, that had an one inch diameter pulley not guarded, thereby exposing the employee to a caught-in hazard.  The pulley was approximately 36 inches from the floor.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.  a.  At the workplace located at 102 N. Sundale Road, Norwich, Ohio, employees operated a Herhammer wick tabbing machine, that had a horizontal belt not guarded, thereby exposing the employee to a caught-in hazard.  The belt was approximately 38 inches from the floor.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a.  At the workplace located at 102 N. Sundale Road, Norwich, Ohio, employees operated a Herhammer wick tabbing machine, that had a chain and sprocket not guarded, thereby exposing the employee to a caught-in hazard.  The sprocket was approximately 30 inches from the floor.
