29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) In the production area, on or about and at times prior to May 17, 2017, the employer did not ensure employees who were required to operate the Techma Ironworker shear machine, to shear small parts such as, but not limited to washers, and by holding the part with their hands, employed a tool to prevent employee exposure to the point of operation, exposing employees to amputation hazards.  b) In the production area, on or about and at times prior to May 17, 2017, the employer did not ensure employees who were required to operate the Techma Ironworker punch press, to punch holes in parts, were safeguarded by a guard at the point of operation, exposing employees to point of operation hazards.
