The employees were not trained to know at least the following: When PPE is necessary; What PPE is necessary; How to properly don, doff, adjust, and wear PPE; The limitations of the PPE; and, The proper care, maintenance, useful life and disposal of the PPE.     a) Facility located at 6950 Sherman Lane, Pennsauken NJ, Warehouse:  The employer failed to provide training to employees on when to use the personal protective equipment, why the selected personal protective equipment is needed and its limitations, how to properly wear, adjust and care for the selected personal protective equipment, on or about 5/17/2015. Employees were allowed to wear regular prescription glasses with polycarbonate lenses, on or about 5/17/2017.
