29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:     a.  On or about May 17, 2017, at the above addressed jobsite, an employee in a hoist area was exposed to falls of approximately thirty (30) feet or 3 stories created by the presence of unprotected sides, thereby exposing the employee to the hazards associated with falls.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a.  On or about May 17, 2017, at the above addressed jobsite, employees conducting masonry operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.
