29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:     a. On or about May 16, 2017, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to clean in and around the Amada horizontal band saws, and are exposed to amputation hazards from the saw and table in that the equipment was not properly deenergized.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     a.)    On or about May 16, 2017, band saw operators were exposed to amputation injuries in that the Amada horizontal band saws, including but not limited to the #5 Amada, were not locked out prior to cleaning. The employer did not ensure that energy control application steps to control the electrical and hydraulic energy sources were implemented prior to cleaning tasks  the employer did not:   �  Physically operate energy isolating devices to control energy sources and affix lockout device(s).   �  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.   �  Verify that isolation and deenergiziation of the machine or equipment had been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.)    On or about April 5, 2017, the employer did not ensure employees were protected while operating the #5 Amada horizontal band saw. Employees were required to use the band saw for cutting long stock pieces, and are exposed to amputation hazards from the pinch point of the moving table that holds the stock.      b.)    On or about May 16, 2017, the employer did not ensure employees were protected while operating the Amada horizontal band saws, including but not limited to the #5 bands saw. Employees were required to work around the band saws and are exposed to amputation hazards from the unguarded moving saw dust augers.

On or about May 16, 2017, the employer did not ensure the hospitalization that occurred on April 5, 2017, was reported to the Toledo Area Office within 24 hours of the incident.
