29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about May 10, 2017, on the steep roof of a multi-family dwelling, employees were exposed to a fall hazard exceeding six feet when performing roofing work without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 10, 2017, on the North side of a multi-family dwelling where employees were exposed to a fall hazard while using an extension ladder to access a roof without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing or other rigid support.
