29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation.  a) South Exterior Wall, 72 Washington Street, East Stroudsburg, PA - Employees installed wood from the scaffold platform of a supported frame scaffold with one leg of each bottom frame (near the wall) placed on dirt/gravel and one leg of each bottom frame (near the road) placed on a cinder block, exposing employees to an unplanned collapse of the scaffold, on or about May 17, 2017.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Main Entrance Overhang Roof, 72 Washington Street, East Stroudsburg, PA - Employees installed rubber coating on a low sloped commercial roof without any fall protection and were exposed to a fourteen-foot (14') fall hazard, as observed on or about May 17, 2017.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) East Exterior Wall, 72 Washington Street, East Stroudsburg, PA - Employees used a Model FE3228 Louisville Fiberglass Type IA Extension Ladder for roof access/egress with side rails that extended twenty four-inches (24") above the roof eave exposing the employees to a fourteen-foot (14') fall hazard, as observed on or about May 17, 2017.
