29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: Gangway 3, landing: Sections on both the left and right side of the gangway landing had inadequate standard railings; the right side was missing a section of top and mid rail completely and the left side had slack rope used in place of the railings. Employees access the ship daily and are exposed to a fall hazard in excess of fifteen feet.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: USS Hornet, Maintenance Shop: The following pieces of equipment were not provided with safe guarding to prevent unintentional contact: 1. Monarch #14AA, horizontal lathe, did not have a guard for the rotating chuck. 2. Drill press, floor mounted, did not have a guard at the point of operation.
