29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:    On or about May 18, 2017, at the worksite located at 8104 Northshore Drive in Benton, Arkansas, employees using pneumatic nail guns to fasten shingles to the roof did not wear eye protection.  This condition exposed employees to the hazard of eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about May 18, 2017, at the worksite located at 8104 Northshore Cove in Benton, Arkansas, employees on a steep pitched roof engaged in roof shingling activities were exposed to unprotected falls of greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about May 18, 2017, at the worksite located at 8104 Northshore Cove in Benton, Arkansas, the Werner 24 ft. extension ladder used to access and descend from the roof did not extend 3 feet above the landing surface.  This condition exposed employees to falls from elevations.
