29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                             A) Exterior, On roof: An employee was exposed to eye/face injuries while using an electric saw and was not protected with any type of eye/face protection, on or about April 14th, 2017.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   A) Interior, From roof to the scaffold: Employees were exposed to fall hazards of approximately 18 feet while working on a fabricated framed scaffold that was not provided with safe acceptable means of access, on or about April 14th, 2017.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:   A) Building exterior, outside of the lift:  An employee was working in an JLG lift and was climbing on the railings of the lift and working outside of the lift on the roof (while tied off to the lift), therefore exposing the employee to fall hazards of approximately 40 feet, on or about April 14th, 2017.
