29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. On or about May 19, 2017, and at times prior thereto, the Baldor bench grinder had the work rests adjusted approximately 2 1/4 inches on the right side and 1/2 inch in the left side, exposing employees to the hazard being struck by flying wheel fragments in the event of wheel breakage.
