29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: On or about May 19, 2017, employees were exposed to struck by hazards. The employees were performing roofing activities on an existing residential home located at 311 Chestnut, Cuyahoga Falls, Ohio 44221. The employees were using air powered nail guns without wearing safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May 19, 2017, employees were exposed to a fall of approximately 22 feet. The employees were performing roofing activities on an existing residential home located at 311 Chestnut, Cuyahoga Falls, Ohio 44221. The employees were working on all areas of the roof without using any form of fall protection.
