29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   On or about 05-16-2017 and times prior thereto, located at U.S.C.G. Base Elizabeth City, 1664 Weeksville Rd. Building #48, Elizabeth City, NC 27909, employee(s) were exposed to unguarded and unprotected fall hazards greater than 14 feet when assigned to cover an approximate 5 foot by 20 foot roof opening with plastic that was left uncovered by commercial contractors .  The opening was located on the rooftop of the USCG Fire Department.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section.  On or about 05-16-2017 and times prior thereto, located at U.S.C.G. Base Elizabeth City, 1664 Weeksville Rd. Building #48, Elizabeth City, NC 27909, employee(s) were exposed to unguarded and unprotected fall hazards greater than 14 feet through a hole in the rooftop when assigned to cover the hole that was approximately 5 foot by 20 foot in size.  The hole was located on the rooftop of the USCG Fire Department.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about 06-01-2017 and times prior thereto, located at USCG Base Elizabeth City - 1664 Weeksville Highway Elizabeth City, NC 27909, Building #48, employee(s) were exposed to fire and electrical hazards where an approximate 100 foot temporary extension cord that was run through walls and ceilings was supplying power to a MOVINCOOL Classic Plus 14 Spot Cooling Air Conditioning system equipped with a UL approved LCDI cord and an approved NEMA plug configuration.  The LCDI device was not plugged into an appropriate wall outlet when used with an extension cord, which the manufacturer stated must not be done.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors:  On or about 06-01-2017 and times prior thereto, located at USCG Base Elizabeth City - 1664 Weeksville Highway Elizabeth City, NC 27909, Building #48, employee(s) were exposed to fire and electrical hazards where an approximate 100 foot temporary extension cord was run through walls and ceilings to supply power to a MOVINCOOL Classic Plus 14 Spot Cooling Air Conditioning system.  The energized cord came from a cord reel in the apparatus bay, passed through the block wall and down through the false ceiling where it rested upon the metal edge of the false-ceiling frame without chafing protection. Sections of cord were located above the ceiling tile and inside the masonry block wall and was not visible to be inspected.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1904.29(b)(6): The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent.   Date of Violation: On or about 05-16-2017 and times prior thereto, located at USCG Base Elizabeth City - 1664 Weeksville Highway Elizabeth City, NC 27909, Building #48, all employee names on the OSHA 300 logs were removed from the logs for the purpose of protecting Personally Identifiable Information (PII) and not for "Privacy Case" reasons.  The space normally used for employee names was removed entirely.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
