On or about 05-16-2017 and times prior thereto, located at U.S.C.G. Base Elizabeth City, 1664 Weeksville Rd. Building #48, Elizabeth City, NC 27909, employee(s) were exposed to unguarded and unprotected fall hazards greater than 14 feet to concrete ground below through a hole in the rooftop approximately 5 foot by 20 foot in size. The hole was located on the rooftop of the USCG Fire Department and was for the placement of an HVAC system. Abatement certification required

(a) Date of Violation: On or about 05-16-2017 and times prior thereto located at U.S.C.G. Base Elizabeth City, 1664 Weeksville Rd. Building #48, Elizabeth City, NC 27909, on top of building #48 Fire Station, employee(s) conducting roofing repair work of installing metal "curbing" was exposed to falls from heights approximately 14 feet, 4 inches above the concrete ground level below without any type of fall protection system, fall restraint system, safety net, guardrails, warning system, safety monitoring system, or any combination thereof.
