29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:  a) 208 Freedom Circle, Kulpsville, PA: On or about May 17, 2017, three employees were working below other workers, who were handling building materials and using tools, without utilizing any means of head protection.      b) 203 Revere Circle, Kulpsville, PA: On or about May 17, 2017, an employee worked below other workers, who were installing roofing materials and using tools, without utilizing any means of head protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) 203 Revere Circle, Kulpsville, PA, Roof: On or about May 17, 2017, employees used a 24-foot extension ladder to access a residential, approximately 20 feet above grade, that did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard of no less than 20 feet.
