29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. This violation occurred on May 22, 2017 at 2300 N. Lincoln Blvd, Oklahoma City, Oklahoma worksite: Employees were using fabricated tubular frame sectional scaffold to access the roof which had not been inspected by a competent person before work began. Employees were exposed to the type of hazards associated with falls due to scaffold failure.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. This violation occurred on May 22, 2017 at 2300 N. Lincoln Blvd, Oklahoma City, Oklahoma worksite: Employees using fabricated tubular frame sectional scaffold to access the roof were not adequately trained to recognize the hazards and to control or minimize the hazards associated with the type of scaffold in use exposing employees to fall hazards.
