Employees perform tasks such as (but not limited to) unjamming equipment and roll changes, and the employees have not been trained in safe application, usage and removal of controls or what procedures should be followed.

Employees that work in the area where the baler, presses and folders are operated do not use hearing protection and retraining had not been provided to the employees to ensure they understood when the equipment was necessary to be used.
