29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  a) On or about April 3, 2026, in the Press Room, ABG Digicon Digital Printing Press operators were not provided with appropriate hand protection while handling Nazdar OP2015 UV Matte Varnish ink. Employee hands/arms were exposed to contact with a skin sensitizer (Category 1) when cleaning excess ink from the rollers and the ink pan.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about April 1, 2026, in the Press Room, the employer failed to ensure written lockout tagout procedures for maintenance and servicing activities on the HP Indigo WS6000 Series Digital Press were developed and utilized. Employee's hands and arms were exposed to pinch point and entanglement hazards created by surrounding gears, flywheels, and other mechanical parts while performing maintenance activities such as replacing the heat exchanger.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) On or about April 3, 2026, in the Press Room, the employer failed to ensure that employees that handled hazardous chemicals such as adhesives, oils, and production of Ozone during digital printing operations were provided with an effective training program on the recognition, avoidance, and mitigation of exposure to hazardous chemicals. The presence of hazardous chemicals exposed employees to significant health hazardous such as but not limited to skin sensitization, respiratory tract damage (chronic and acute), carcinogens, and eye irritants.
