29 CFR 1926.651(i)(1): Where the stability of adjoining buildings, walls, or other structures was endangered by excavation operations, support systems such as shoring, bracing or underpinning was not provided to ensure the stability of such structures for the protection of employees:  a) Jobsite, 208 & 210 Country Drive, Belle Vernon, PA:  On May 22, 2017, a French drain was uncovered while excavating for the installation of a new storm sewer, was not supported, the previously disturbed soil surrounding it became loose and fell down into the trench along with other soil from that side of the trench, resulting in the trench collapse.  There also were landscaping rocks on the edge of the excavation, directly above the area where the French drain was located, adding additional weight to the side of the trench.  No measures were taken to shore, brace or support this area.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:  a) Jobsite, 208 & 210 Country Drive, Belle Vernon, PA:  On May 22, 2017, the competent person did not remove two employees from a 9 foot deep trench that had previous indications of a hazardous situation that did result in a cave-in.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  a) Jobsite, 208 & 210 Country Drive, on May 22, 2107, two employees were installing a new storm sewer in an unprotected trench that was approximately 9 feet deep when it collapsed which resulted in one fatality.  The company Superintendent was present on the job site and was aware that the employees were working in the unprotected trench.  R. A. Monzo Construction Company, Inc. was previously cited for a repeat violation of this occupational safety and health standard which was contained in OSHA inspection number 312632797, citation number 1, item number 1, issued on May 5, 2009, with respect to a workplace located at Sweden Alley, Irwin, PA  15642.  R. A. Monzo Construction Company, Inc. was previously cited for a repeat violation of this occupational safety and health standard which was contained in OSHA inspection number 309040996, citation number 2, item number 3, issued on June 6, 2006, with respect to a workplace located at Raley and State Street, Belle Vernon, PA  15012.  R. A. Monzo Construction Company, Inc. was previously cited for a serious violation of this occupational safety and health standard which was contained in OSHA inspection number 308013341, citation number 1, item number 3, issued on March 15, 2005, with respect to a workplace located at Gettemy Road, Greensburg, PA  15601.  R. A. Monzo Construction Company, Inc. was previously cited for a serious violation of this occupational safety and health standard which was contained in OSHA inspection number 304179393, citation number 1, item number 5, issued on July 19, 2001, with respect to a workplace located at 500 Sarver Road, Sarver, PA  16055.
