29 CFR 1910.178(l)(4): An evaluation of each powered industrial truck operator's performance was not conducted at least once every 3 years. On or about May 24, 2017, the employer failed to ensure that each authorized forklift operator was competent to safely operate powered industrial vehicles. Forklift operators with expired licenses had not been retrained or evaluated. Employees were exposed to the hazards associated with unverified operators using powered industrial vehicles to load and unload materials in trucks and within the warehouse. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
