Employees were exposed to injury through a scaffold collapse as concrete blocks, which are not an adequate foundation, were used to support one of the fabricated frame scaffolds and other scaffold poles lacked base plates. Violation observed on or about 05/20/2017.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) 714 Route 10, Randolph, NJ. Employees were exposed to falls of up to 19 to 20 feet to the ground below as workers involved with exterior wall masonry work were using the scaffold frames and crossbraces as a means of access from one level to the other. Violation observed on or about 05/20/2017.
