29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     On or about May 23, 2017, employees who performed service and maintenance activities on or around the company's powered presses were not required to:     1. prior to the start of any work activity on machinery and/or equipment that had been lock out or tagged out, verify that isolation and deenergization of the machine or equipment had been accomplished.      Although the employer developed and enforced the use of energy control procedures, they were deficient in that, verification of isolation [(d)(6)] steps were not included or applied to protect employees from machine servicing hazards from unintended start-ups and energizing of the powered presses.
