29 CFR 1926.152(d)(2): A portable fire extinguisher having a rating of not less than 20-B units was not located between 25 feet and 75 feet from flammable liquid storage area(s) located outside: a) On Site, near the General Contractor's construction trailer, a 1000 gallon storage tank containing 1993 dyed diesel used to fuel equipment was not equipped with a fire extinguisher, exposing employees to fire hazards. b) On site, on the Old Dowlen Road Expansion site, a 500 gallon storage tank containing 1993 dyed diesel used to fuel equipment was not equipped with a fire extinguisher, exposing employees to fire hazards. c) On site, on the Old Dowlen Road Expansion site, a 500 gallon storage tank containing 1993 dyed diesel used to fuel equipment was not equipped with a fire extinguisher, exposing employees to fire hazards. d) On site, on the Old Dowlen Road Expansion site, a 500 gallon storage tank containing 1993 dyed diesel used to fuel equipment was not equipped with a fire extinguisher, exposing employees to fire hazards.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a) On May 23, 2017, at the old Dowlen Road expansion, storm water box 1 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. b) On May 23, 2017, at the old Dowlen Road expansion, storm water box 2 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. c) On May 23, 2017, at the old Dowlen Road expansion, storm water box 3 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. d) On May 23, 2017, at the old Dowlen Road expansion, storm water box 4 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. e) On May 23, 2017, at the old Dowlen Road expansion, storm water box 5 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. f) On May 23, 2017, at the old Dowlen Road expansion, storm water box 6 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. g) On May 23, 2017, at the old Dowlen Road expansion, storm water box 7 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically. h) On May 23, 2017, at the old Dowlen Road expansion, storm water box 8 presented impalement hazards to employees working in the area due to uncapped rebar sections measuring 8" to 11" in length vertically.
