29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut: 686 South Main Street, Lanesboro, MA: On or about April 27, 2017 employees were using a Rigid Portable Table Saw Model TS2400-1 Serial#U054897435 to cut PVC trim pieces which had a blade that was not guarded by a hood which completely enclosed the portion of the saw above the table and that portion of the saw above the material being cut exposing employees to amputation hazards. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1904.39(a)(2): The in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident was not reported within 24 hours. 686 South Main Street, Lanesboro, MA: On or about May 17, 2017 the employer had not reported an employee's amputation as a result of a work related accident which occurred on April 27, 2017 to either the local area office, National Hot line number or the electronic serious injury report via OSHA.gov as required.
