OMNI Baking 2621 Freddy Lane, Vineland, NJ : On or about 5/24/2017 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2017. a) Production, White Bread: An employee working on the line loss consciousness on May 1, 2017. The incident was not recorded on the OSHA 300 form. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
