29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a) At the jobsite, on or about 5/24/17:  Employees were working from a job built platform elevated on the forks of a rough terrain fork-truck.  This platform was moved horizontally from the second story deck where the workers stepped onto it, to the side of the building at the same level where they were working when observed.  This platform was not secured to the forks or mast of the fork-lift.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) At the jobsite, on or about 5/24/17:  Employees were working from a ladder-jack scaffold, 16 feet from ground without fall protection of any kind.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) At the jobsite, on or about 5/24/17:  Employees were working from a job built platform elevated on the forks of a rough terrain fork-truck, at 10, 12, and 16 feet from ground without fall protection of any kind.  b) At the jobsite, on or about 5/24/17:  Employees were working and transiting on a two-level deck system that was 8 feet 4 inches and 7 feet 7 inches from ground without fall protection of any kind.    c) At the jobsite, on or about 5/24/17:  Employees were working on the main level of the building and using the stairwell into the basement level where that stairwell did not have guardrails, exposing employees to falls of 8 feet to the basement level

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) At the jobsite, on or about 5/24/17:  Employees were accessing the deck system at the front of the building on an extension ladder that did not extend at least 3 feet from its upper landing at 7 feet 7 inches.
