OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls in excess of 55 feet from a rope descent system.   On May 18, 2017, at the State Street Bank located at 801 Pennsylvania, Kansas City, Missouri. A Written Plan of Service was not readily available, in that the Plan of Service had never been developed by the employer. The plan shall include but not limited to the identification of hazardous areas, drop zones, safety features etc. The plan must be readily available for use by the building owners or their operating agents, window cleaners, enforcing authorities and emergency personnel.
