29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Jobsite, 915 Brush Creek Road, Warrendale, PA: On or about May 17, 2017, an employee was caulking around windows on a scaffolding that did not have midrails on the ends of working platform. The fall hazard to the employee was approximately 20-25 feet above the ground.
