29 CFR 1926.451(c)(1)(ii): The top guy, tie, or brace of completed scaffolds were placed further than the 4:1 height from the top.  Such guys, ties and/or braces were not installed at each end of the scaffold and/or at horizontal intervals not to exceed 30 feet (9.1 m):  a) Jobsite, 915 Brush Creek Road, Warrendale, PA:  On or about May 17, 2017, employees were working from a scaffold that was approximately 44 feet high and was not tied into the structure until the top of the scaffold.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a) Jobsite, 915 Brush Creek Road, Warrendale, PA:  On or about May 17, 2017, employees were working on a scaffold that was approximately 44 feet high that the base plates were not firmly planted on the mudsills, creating an unstable base.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) Jobsite, 915 Brush Creek Road, Warrendale, PA:  On or about May 17, 2017 an employee was working, alone, on a 6 foot x 8 foot section of roof, 44 feet above the ground, without any type of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite, 915 Brush Creek Road, Warrendale, PA:  On or about May 17, 2017, an employee was working on the roof, without fall protection, and was not trained to recognize the dangers of falling or how to protect himself from falling.  The fall hazard to the employee was 44 feet to the ground.
