29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    (a) Boiler Room - On or about June 29, 2017  two employees were not provided all the necessary PPE while working with corrosive liquids.  Employees are required to move 55 gallon drums that weigh up to 455 lbs.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    (a)    JT Shannon Lumber Company - On or about June 27, 2017 Employees were exposed to caught-in/caught between hazards when conducting servicing and maintenance on equipment such as, but not limited to Cantek Canem - 12 machine, in that; the employer did not establish an energy control program which consisted of an energy control procedure, employee training, and periodic inspections.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.  (a)  JT Shannon Lumber Company - On or about June 29, 2017 the employer did not train new employees in the use of portable fire extinguishers.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  (a)  JT Shannon Lumber Company - On or about June 19, 2017 a Nissan 15500 Capacity Forklift Serial No. 23 had a broken head light and was not reported or corrected. The forklift was driven for at least 2 weeks without being corrected.   This forklift was sometimes used on night shift.
