29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    Worksite at 514 N 3rd St. Okarche, OK 73762. On or about May 24, 2017 employees of a subcontractor were working on the ground below roofing activities and not provided with protective helmets. The subcontractor's employees were exposed to struck by hazards of falling tools and materials overhead.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Worksite at 514 N 3rd St. Okarche, OK 73762. On or about May 24, 2017 employees of a subcontractor were using a portable ladder to gain access to the roof and the ladders side rails did not extend 3 feet above the landing surface, exposing the subcontractor's employees to a fall hazard of more than 6 feet.
