Outer Forty, Chesterfield, MO, had not trained or evaluated their Power Industrial Truck (PIT) operator. The operator had never been trained by Crown Diversified Industries Corporation concerning their PIT (a CAT forklift, Model: 2C5000, S/N: AT9040406).
