29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices and protective shields and barriers were not provided, used, and maintained in a sanitary and reliable condition  wherever it was necessary by reasons of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    (a) On or about 05/25/2017, at facility, for the drivers washing the trucks with corrosive chemstation 90177 containing chemicals such as, but not limited to, glycolic acid and formic acid.  The employer did not enforce the use of personal protective equipment for the protection of the extremities such as, but not limited to, face shield, apron, and safety glasses.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    (a) On and before 05/25/2017, at facility, for the drivers filling the trucks with up to 200 gallons of water while sand, dry concrete, fly ash, and slag dust spills out of the truck and onto the employees containing chemicals such as, but not limited to, silica.  The employer did not perform a hazard assessment in the event that the truck is not lined up exactly under the chute or the blower at the top of chute is not working to determine the appropriate PPE or administrative and/or engineering controls needed to protect the employees.
