29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a) On or about May 25, 2017, and at times prior, the employer did not ensure that a worksite specific written respiratory program was available and implemented in the establishment in Aurora, CO when the use of respirators was required during slab fabrication procedures.    Stoneworks of Colorado, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134 Respiratory protection, which was contained in OSHA inspection number 510318, citation number 1, item number 2 and was affirmed as a final order on December 18, 2012 , with respect to a workplace located at 3555 S Lincoln Ave, Unit A, Loveland, CO 80537. 29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) On or about May 25, 2015, and at times prior, the employer did not provide a medical evaluation to each employee required to wear 3M tight fitting half-face respirators with 2091 P100 filters to perform stone fabrication operations.  Stoneworks of Colorado, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134 Respiratory protection, which was contained in OSHA inspection number 510318, citation number 1, item number 2 and was affirmed as a final order on December 18, 2012, with respect to a workplace located at 3555 S Lincoln Ave, Unit A, Loveland, CO 80537. Stoneworks of Colorado, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200 Hazard communication, which was contained in OSHA inspection number 510318, citation number 1, item number 7 and was affirmed as a final order on December 18, 2012, with respect to a workplace located at 3555 S Lincoln Ave, Unit A, Loveland, CO 80537.
