29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a) GDB International, Inc./New Brunswick, NJ- Warehouse Area: The employer did not ensure that bundles of recycled plastic were stacked securely. The bundles were stored haphazardly and were sliding in different directions. Condition observed on or about May 25, 2017.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): GDB International, Inc./New Brunswick, NJ- Warehouse Area: The employer authorized the use of forklifts which had visible defects. Employees operated the following forklifts on daily basis: a) Toyota (Model 8FGCU25, Serial 76343) which had visible damage to the front guard b) Toyota (Model 8FGCU25, Serial 76711) which had visible damage to the front guard in addition to the guard not being secured Condition observed on or about May 25, 2017.
