29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At 6061 Colonial Parkway, Club House, Gulf Shores, Alabama: On or about May 24, 2017 and a times prior; the employer allowed his employees to perform painting activities from a high slope roof without fall protection exposing his employees to a fall hazard of up to 11.11 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 6061 Colonial Parkway, Club House, Gulf Shores, Alabama: On or about May 24, 2017 and a times prior; the employer allowed his employees to use an extension ladder to gain access to the roof and the rails of the ladder did not extend at least three feet above the landing to provide safe access exposing his employees to a fall hazard of up to 11.11 feet to the ground below.
