OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. a) Machine Shop Horizontal Lathe #3 - On or about May 24, 2017, an employee was exposed to caught in and amputation hazards while performing sanding and polishing procedures on a rotating shaft by holding emery cloth on the spinning surface with a gloved hand. ABATEMENT NOTE: Feasible means of abatement include but are not limited to includes the following: 1) Provide an industry accepted polishing tool or sanding device that can be mounted on the lathe carriage to eliminate the need for employees to have their hands touch, or enter into the danger zone of the spinning part. 2) Perform and publish a Job Hazard Analysis (JHA) to identify the specific machine hazards associated with individual items that are machined on the lathe. The JHA should identify protective measures and specific machine hazards that may occur. 3) Define and publish specific work rule procedures to guide employees in the proper set up, and operations required to machine specific parts on each lathe. Work rules should define procedures such as the proper securing of the parts using Headstock Chuck, Tail Stock Center, Steady Rest, and Carriage. Procedures also need to define Headstock RPM Speeds, and Carriage Feed Rates. Procedures should address use of appropriate PPE and the exclusion of inappropriate use of items such as gloves, jewelry, long hair, and loose fitting clothing. 4) Train employees on the specific JHA and work rules and ensure that the procedures are being followed through worksite evaluations.
