29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation:    a) Throughout Yard/Facility - On or about May 15, 2017 and times prior to, the employer did not ensure that all employees who operated powered industrial trucks (PITs) had been trained to recognize and understand the hazards involved in using a PIT to unload slab lift clamp attachments from trailers without using the fork slots on the clamp, thus exposing employees to struck-by hazards.

29 CFR 1910.178(n)(7)(iii):  On all grades the load and load engaging means were not tilted back if applicable, and was raised more than necessary to clear the road surface:    a) Yard - On or about May 15, 2017, while the JCB 520 Loadall Telehandler was traveling across the yard to pick up a load, the operator raised the load engaging means more than necessary to clear the surface of the ground on which it was traveling, thus exposing employees to struck-by hazards.
