29 CFR 1926.403(b)(1)(ii): Safety of equipment was not determined on the basis of mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: On or about May 25, 2017, and in times prior thereto, a job made dead front was attached to the face of a breaker panel with black tape, located at the opening to the construction site. This condition exposed workers to the hazard to electric burns in the event the metal sheet created a short circuit.
