29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On or about May 23, 2017, at the job site an employee was observed working in a shop made platform which was mounted on an all-terrain forklift truck elevated to a height approximately 14 feet high. The employer did not have a written approval from the forklift truck manufacturer to allow such addition/modification to the forklift. Employees were exposed to fall and struck by hazards.
